Excessive Straining During Bowel Movements May Put Added Stress on Heart, Doctors Warn

Mumbai, Sep 2026 : Excessive straining while passing stools may not only aggravate constipation but could also place sudden stress on the cardiovascular system and, in vulnerable individuals, potentially trigger a serious cardiac emergency, doctors have cautioned.

Dr Bipeenchandra Bhamre, cardiac surgeon at Sir HN Reliance Foundation Hospital, said the risk could be particularly significant for older adults and people living with high blood pressure, diabetes or pre-existing heart disease.

According to the doctor, forceful straining during bowel movements can temporarily alter blood pressure and heart rate, increasing the workload on the heart. While such changes may not cause problems in healthy individuals, they could become dangerous for people with underlying cardiovascular conditions.

“This sudden strain temporarily affects blood pressure and heart rate, placing extra stress on the heart,” Bhamre said. He added that people with heart disease, uncontrolled hypertension, diabetes or weakened heart function could experience complications ranging from chest pain and abnormal heart rhythms to, in severe cases, a heart attack.

The pressure generated during forceful straining can also extend beyond the digestive system. Increased pressure inside the chest may further burden the cardiovascular system, particularly in people whose heart function is already compromised.

Doctors said symptoms of a cardiac event can appear suddenly and should not be ignored. Warning signs may include intense chest pain or pressure, discomfort spreading to the left arm, neck, jaw, shoulder or back, breathlessness, cold sweating, dizziness, nausea, vomiting, unusual fatigue or sudden collapse.

Bhamre advised people experiencing chest pain or severe discomfort while passing stools to stop straining immediately and seek medical assistance.

“If chest pain or severe discomfort starts while passing the bowels, then don’t strain, stop immediately, and ask for help,” he said, urging people to consult a doctor without delay.

Doctors also stressed that preventing constipation can reduce the need for forceful straining. Adequate water intake, a fibre-rich diet and regular physical activity can support healthy bowel movements. Around 45 minutes of daily exercise may also contribute to better cardiovascular health.

People with existing heart conditions should take their prescribed medicines regularly and maintain control over blood pressure, blood sugar and cholesterol levels. Routine medical check-ups are also important for identifying and managing cardiovascular risks.
